The essentials of nursing assessment and care planning in a care home: a case study

Abstract

Nurses play a key role in the assessment and planning of patient care. Using a case study approach, this article discusses the importance of a person-centred approach in identifying a care need, determining a goal of care and providing evidence-based nursing interventions.
