Abstract

Stigma associated with mental illness often extends beyond public and healthcare domains into familialrelationships, creating new challenges for individuals within their immediate families. This study explores the livedexperiences of family-context stigma among individuals diagnosed with severe mental illnesses, aiming tounderstand how stigma within familial relationships manifests, its impact on psychological well-being andempowerment, and strategies for addressing and resolving it. Grounded in the understanding that stigma is a socio-cultural construct, the research highlights how family-context stigma often mirrors societal attitudes, creatingadditional challenges for individuals. Using a qualitative design, 20 participants from Northern Ireland wereinterviewed, providing in-depth insights into the multifaceted nature of family-context stigma. The findings revealthat family-context stigma manifests as relational bias, verbal or emotional abuse, exclusion, shame, guilt, andalienation, significantly undermining individuals’ psychological well-being and quality of life. Conversely, momentsof support, acceptance, and proactive efforts by family members to challenge stigma were identified as criticalmitigating factors. While the study is limited to Northern Ireland and a relatively small sample, it underscores theneed for future research with larger, diverse populations to validate these insights. The findings emphasize theimportance of open communication, mental health education, and fostering empathetic relationships within families. These insights have implications for designing resources, developing targeted interventions to reduce familialstigma and inform policies and programs that promote inclusive, supportive family environments.
